Phyllis was born in Heanor, Derbyshire, England in 1892.
The 1901 census is a little bit confusing as it has Phyllis as head of the house at 9 years old. A number of servant are living at the property, so her parents were probably traveling.
A Nurse in the VAD she landed in France on the 9th June 1915, serving there until the 9th of January 1916.
She later married and died in 1942 aged 50.
